The following forms are provided for the use of APPX Resellers, in support of their customers. These forms are Adobe Acrobat form files, which may be completed electronically and/or printed out with Adobe Acrobat Reader software (free download from Adobe.com).

Sales Report Form – New Sales – To be completed and signed by VAR for all new sales, defined as sales to new customers or additional CPUs for existing customers.

Platform Change Request Form - To be completed and signed by Customer for transferring a license from one operating system platform to another (e.g., from Windows® to Linux, or from HP-UX to Solaris). Moving to a newer version of the same OS does not require a Platform Change, but may require a Registration Change if the serial number is different.

Registration Change Request Form – To be completed and signed by Customer for registration changes, including change of customer name, CPU Location, serial number, etc.

Temporary Registration Request Form – To be completed and signed by VAR for all temporary registrations, including demo, evaluation, emergency replacement, extensions, etc.

Price Quote Request Form – For VARs that would like a complete and accurate price quote before presenting a proposal to a customer, just fill out the form and send it in to us. We'll send back the list prices for you and your customer to review.

Software Evaluation Confidentiality Agreement – This Agreement is used whenever a VAR provides a copy of any Authorized Software and Authorized Application to a potential customer for evaluation for a period not to exceed sixty days.

Credit Card Authorization Form - APPX accepts credit card payment for many transactions. Complete, print, and sign this form, and fax it back to us at 1-904-880-6635.
